change data of file and image cards to URL

Sizes can be specified via size: in inclusions and size= in URLs - hopefully that "just works" when this is done, otherwise I guess we can ticket it.

this is not exactly what we're doing. the new solution is described in implement password security for files. Basically, all actual files will now be behind a security wall, which means that they can only be referred to by the cardname. This means the card's raw content actually becomes irrelevant to regular rendering. Therefore closing.
