rename relative inclusions in forms

When you rename a card that is used as a relative inclusion in forms, the form is not changed to reflect this. I get bit by this not infrequently. --John

yes, ok, yes. thanks, john. I think the problem is that we're not storing the reference from the content form. It probably needs special handling. If I'm on user+*type+*content and I include +email, then this should probably be stored as a reference to "email", not user+*type+*content+email.
